I have read and accepted the External Product Provider Agreement and the End User License Agreement.

1. To read the agreement - External Product Provider Agreement and the End User License Agreement.

2. Complete and execute Direct Debit Request and Payment Approval (V-Pay) Form, complete the IntegraPay application. By completing and executing the Direct Debit Authority and Payment Approval (V-Pay) form you authorise direct debit of the following payments, which are described in more detail in your External Product Provider Agreement:

• Combined Booking Fee of 2.8% on the total Online Booking value at the time of Online Booking.

• Distribution Fee at the Distributor Rate as agreed by you when you opt into a Distributor.

• Repayment by you of any over-payments.

3. Once your Application is processed an email will be sent to you to create a new Stripe account or link your existing Stripe account. Stripe will charge a per transaction fee as detailed at https://stripe.com/au/pricing. An additional Payment Services Fee of 0.82% (ex GST) is applied to each Online Booking utilizing the Stripe services..
